Not licensed in the State of Arizona yet? The forms below are what you need to print and complete to start the process in becoming a Licensed Interpreter.
Please note that the application fee is non-refundable. We only accept money orders, cashier’s checks, personal checks, payable to “Arizona Commission for the Deaf and the Hard of Hearing (ACDHH)” or cash (in person).
The Governor’s Regulatory Review Council approved changes to the Licensure Rules on June 7, 2016, which are effective as of August 15, 2016. A draft of the rules is available here:
