As of today, April 20, 2018 after 5 p.m. through April 25, 2018 – we will be transitioning to a new e-mail system. We hope the transition will be a smooth process, however there is a  possibility of loss of some emails. Currently licensed? The renewal forms are below.

NOTE: Your renewal application must be received in the office on or before your expiration date but no earlier than 60 days prior to your licenses expiration. Once your renewal application has been received by the ACDHH office, it is processed within 90-days. Licensees can continue to work while their renewal application is being moved through process. Should verification for an employer be needed, please feel free to contact the Licensure Department.

Please note that the application fee is non-refundable. We only accept money orders, cashier’s checks, personal check, payable to “Arizona Commission for the Deaf and the Hard of Hearing (ACDHH)” or cash (in person).

Suggestion: To fill out form electronically, save the downloaded forms to your computer, open it and fill it out and save.

Make sure your application is complete, filling each field, each box checked off and submit supporting documentations. Any fields or supporting documents can delay your application process. Please read the application carefully. Thank you.
