AZTEDP is continuing to take applications for equipment. Both gently used and new devices are available at this time. Due to the current Arizona state budget reductions, our first priority is to utilize inventory on hand. If necessary telephone equipment demonstrations are available at the ACDHH/AzTEDP office in Phoenix by making and appointment.
Application Instructions:
- COMPLETE the application form. The AzTEDP application form must be completed by the applicant in part A. It also needs to be FILLED OUT AND SIGNED by a vertifying professional in Part D.
- COMPLETE the Arizona Statement of Citizenship Form. Include a legible COPY of supporting documentation with completed form. For most people this is a passport or birth certificate in their current name. If their name has changed they must provide the appropriate documentation, such as a marrage license. (See pages 3-6 of Citizenship form for more options to prove legal presence in the United States). Arizona Revised Statutes §1-501
- Provide proof of Arizona residency, such as Drivers License, State ID Card, or utility bill in the applicants current name and address.
- COMPLETE the Conditions of Acceptance form by signing at the bottom.
- COMPLETE the Equipment Preference sheet.

You will be responsible for plugging in the equipment. Please read the manual. For Hard of Hearing users, PLEASE BE SURE THAT YOUR AMPLIFIER IS TURNED ON.
Arizona Telecommunications Equipment Distribution Program (AzTEDP)
The State of Arizona loans telecommunications devices at no cost to resident of the State who need assistance communicating on the phone.

AzTEDP is the free, state-funded program that loans telecommunications devices to qualified residents with hearing loss who need phone assistance.
AzTEDP Devices Include:

- Amplified Telephones
- CapTels
- TTYs
- Be ready to provide proof of Arizona residency (state-issued ID card, driver's license or utility bill with your current address).
- Once your application is complete, a certified professional must sign it, verifying your hearing loss.
Note: The application process takes about four-to-six weeks. Incomplete applications can cause a delay in the process.
When do you receive your equipment?
After all forms and photocopied documentation has been mailed to AzTEDP, it will be reviewed for completeness. If approved, our first option to serve you will be to ship either of your preferred devices from our refurbished inventory. A voucher and instructions for a new device will only be issued when no preferred inventory is available. If application materials are incomplete, a pending letter will be sent. Applications remain active for six months.
How did AzTEDP begin?
In 1986, the Arizona Commission for the Deaf and the Hard of Hearing began a program to distribute TTYs to Deaf Arizonians so they could access the public telephone system. Today, AzTEDP has grown into one of the most comprehensive telecommunications loan programs in the nation, serving the Deaf, Hard of Hearing, Deaf-Blind, and Speech Impaired residents of Arizona.
