Application
AZTEDP is continuing to take applications for equipment. Gently used devices are available at this time. Due to the current Arizona state budget reductions, new devices are not available. We encourage individuals to continue to apply for the telephone program. ACDHH will begin processing applications/vouchers for NEW equipment in approximately July, 2010. Telephone equipment demonstrations are available at the ACDHH offices in Phoenix.
- COMPLETE the application form. The AzTEDP application form must be completed by the applicant in part A. It also needs to be FILLED OUT AND SIGNED by a vertifying professional in Part D.
- COMPLETE the Arizona Statement of Citizenship Form. Include a legible COPY of supporting documentation with completed form. For most people this is a passport or birth certificate in their current name. If their name has changed they must provide the appropriate documentation, such as a marrage license. (See pages 3-6 of Citizenship form for more options to prove legal presence in the United States)

Provide proof of Arizona residency, such as Drivers License, State ID Card, or utility bill in the applicants current name and address.
