Ongoing training and development
The PROGRAM COORDINATOR III provides direct services including case management and advocacy for Deaf, Hard of Hearing and Deaf-Blind populations in the areas of vocation, and life management in community settings, home residences and/or at job sites. Coordinates and facilitates community training and education classes, crisis/emergency services and maintains necessary case records and client file documentation. Position may from time to time be stressful and require a high demand of performance. May perform other duties as assigned.
ESSENTIAL FUNCTIONS FOR THE PROGRAM COORDINATOR III
Provides direct service to individuals in community settings, home residences and/or at job sites
Provides intake services to include reviewing referral information, completing Case History (biopsychosocial), Intake, Confidentiality, Consent to Treat, Authorization to Release Information, and other forms, presenting information to team at intake meetings, and completing and implementing consumer service treatment plans
Provides support and case management to consumers in the areas of life management and independent living skills
Maintains case records and documentation for consumers; completes required weekly and monthly documentation
Coordinates and facilitates various processes and community education classes
Provides advocacy in the areas of public benefits programs, education, and working with employers and other systems in the community
Coordinates and provides direct Support Service Provider (SSP) services to Deaf-Blind consumers
Coordinates the emergency/crisis screening process. Working with consumers in crisis to determine eligibility for emergency (food, housing, utility and transportation) funding and referring to appropriate community resources
Coordinates and oversees Hearing Aid Bank services
Provides case management to Hard of Hearing persons for consumer resources. Assists senior citizens with life planning, as requested
Implements, reviews and maintains CARF procedures and adheres to DRS and CCS policies and procedures
Participates in training and in-services to enhance professional growth
Provides direct services to individuals in community settings, home residences, and/or at job sites
Provides individual, family, and Adult and Youth Group Counseling (substance abuse/dependence, social skills, anger management, budgeting, Parenting, and Men and Women's Group)
Responsible for the Domestic Violence Education series for Youth
Responsible for the Men's Domestic Violence Program with Perceptions
May drive agency or personal vehicle on company business
MINIMUM QUALIFICATIONS FOR THE PROGRAM COORDINATOR III
Bachelor's in Counseling, Rehabilitation, Social Work or related field
Fluency in American Sign Language and various manual communication modes
2 years of experience working with Deaf, Hard of Hearing and/or Deaf-Blind persons in a professional counseling and/or case management capacity
Obtain State of Arizona Level One Fingerprint Clearance Card and pass drug screening
Valid Arizona Driver License, proof of insurance and Arizona Motor Vehicle Division 39 month Motor Vehicle Report
DESIRED QUALIFICATIONS FOR THE PROGRAM COORDINATOR III
Master's in Counseling, Rehabilitation, Social Work or related field
LPC, LISW, LAC or NBCC
Additional years of directly related experience
